HOLDEN POLICE DEPT.
Police Report Request:
Complete this form and return to HPD with payment to request copies of police reports (including motor vehicle accident reports).
Online request for enforcement regarding speeding, crosswalk, stop sign, and other traffic violation issues around town.
5K Race Application:

Register for the Holden Police Association Annual Partnership 5K Run/ Walk by printing this form. Complete a separate form for each applicant and return to us with payment.

Solicitor Permit:
 Complete the application and bring it to the Holden Police station with payment and positive identification. A background check will be done prior to issuing all permits.
Request for
Parking Ticket Hearing:
Complete the form and return it to the Town Clerk's office in the Town Hall, 1196 Main Street, Holden MA 01520 (white building, 1st floor).

FIREARMS FORMS

LTC and FID Application:
Print this form and bring it with you to our firearms class (see our Firearms Information page for more details). The form is required to apply for a new license or to renew an existing one.
All changes of address must be registered with the Chief of Police in both your old hometown and your new one. Print this form and mail it to both police departments.
Review Board Petition:
If your application was rejected, you may be entitled to appeal by using this form and mailing it to the Firearms Review Board.

Operator's Accident Report:
Follow the instructions on the form and make 3 copies to send to: the police department in the town where the accident occured, your insurance company, and the Registry. Keep the original for your records.
If you were in an accident in Holden and a Holden Police Dept. officer responded to the scene, you may be entitled to a copy of the officer's report. Please read the instructions on the form carefully and return it to HPD with payment.
Handicap
Parking Abuse:
If you have good evidence that someone is abusing/misusing a disabled placard or handicap license plate, you can report it to the RMV. This can be done online or you can print a form and mail or fax it to the RMV.

Change of Address:
You have 30 days after your move to change your address with the Registry. You can complete the transaction online on their website, or print the form and mail it in to them.

You can pay the fines associated with a Mass. Uniform Citation (ticket) online. Please wait 10 days after you get the ticket to allow the registry to update their database.

You may request a copy of your own criminal record from the CHSB (Criminal History Systems Board) by using this form.

SEX OFFENDER FORMS

Info Request:
All Massachusetts residents are entitled to information regarding registered level 2 and 3 sex offenders who live and/or work in their immediate area. For information regarding offenders registered in Holden, complete the form and return it in person to the Holden Police Department. You will be asked for identification when you turn in your request form.

Registration &
Change of Address:
 (SOR Form 1-R). Official registration and change of address form issued by the State of Massachusetts Sex Offender Registry Board, required of all sex offenders. This form is also available at the Holden Police station, where the offender must appear in person to register, and be fingerprinted and photographed. Instructions are separate - see next entry.
